Hi there.....if you wanna save money and do it on ur own....just surf the local Sarawak Council Website or even our very own Veterinary Dept lah. You can get most info from there. Or call them up lor. Speak to one of the ppl from the Veterinary Dept.
I did all that when I took my dog back from USA. By taking all the trouble upon myself, I saved about RM7k. Cos someone did offer to do everything for me door to door from USA to Melaka for RM10k!
I was like SCREW IT! Import permit only RM5 I think all in all including flight and quarantine and vet fees in US, I spend less than RM3k. If you're lazy, then pay someone to do it lor. Cos the amount of paperwork and phones calls....i tell its NO KIDDING....and u know lah malaysian system, MR X say this today, tomorrow MS Z say something totally different. So i have to call a few times just to double confirm the documents they needed. Then have to call a few more times to make sure they have a quarantine space for my dog and a few more times before I catch my flight just to make sure they have someone waiting for me in KLIA to take my dog to quarantine. PHEW......

Ugh, of all places Sarawak. You know Borneo is supposed to be rabies free so to keep it that way, they impose a quarantine of up to SIX MONTHS for dogs going to Sarawak from West Malaysia.
